Requests submitted without documentation are considered incomplete. Please submit your documentation with this form. Documentation should be no more than three years old, in English, and should include the following: evidence of an existing disability/deficit, contextual information, relevant testing, specfic diagnosis(es), and explanatory summary. Examples include, psychological educational evaluation or neuropsychological evaluation.
